Log in to your account and go to the Templates page via the left-hand menu under Forms.
Click Add New Template.

Edit fields like the form name, summary, description, and any other details.
Choose a category for easy organization.
Click Review, then Edit Form to fill in the template using Smart Edit.
Click Save to store the template.
